The reports constitute a receipt and adjustment analysis, updated monthly, of all transactions in patient-payor accounts. The patient census lists all patients with their current locations and other pertinent data. The input documents include the Daily Hospital Census Report (DMH 52), Leave and Return Report (DMH 52A), Daily Patient Intra-Hospital Movement Report (DMH 53), Patient Released-To Data Sheet (DMH 54), Outpatient Visit Summary (DMH 55), Patient Name Control Form (DMH 56), Patient Data Sheet (DMH 57), and ARS Batch Control Form (DMH 58), and other input documents and reports related to ARS.
